The Customer Header page will allow user to Add a Customer.

Customers in iContracts are classified by Customer Type as:
- Vendors if they submit chargebacks like wholesalers and distributors. Any Vendor should have the COT Vendor.
- Buying Groups if they contract for a membership like GPOs and Chains. Any Buying Group should have the COT Buying Group or the COT of the entity (e.g.,Chain).
- Purchasers if they purchase on contracts like GPO members. Purchasers may have any end customer COT designation.
- State Medicaid Programs if you are using the iContracts MedicaidPayer module.
- Sponsors if they manage contracts for the Medicare GAP Program.

Note: The field names preceded by an * are required fields.
Enter required fields below:
Customer Type: Select Customer Type from the drop down menu.
Class of Trade: Select from the COT drop down menu.
Territory: Select a Territory from the drop down menu (refer to Territory Maintenace).
Reference Id: This field contains an Internal / Industry account number for the customer.
Name: Customer Name.
Medicaid Program Type: Select Medicaid Program Type if using the iContracts Medicaid Payer Module. (if Customer Type=State Medicaid Program only)
Medicaid State: Select Medicaid State if using the iContracts Medicaid Payer module. (if Customer-State Medicaid Program only)
Address: Customer Address.
City: Customer City.
State: Customer State.
Zip code: Customer Zip code.
Contact: Customer Contact Name.
Bank Name 1: The Bank which the Sponsor uses. Bank 2 line is provided if needed. (if Customer Type=Sponsor only)
Bank City: The Bank's City. (if Customer Type=Sponsor only)
Bank State: The Bank's State. (if Customer Type-Sponsor only)
Bank ABA: The Bank's routing number. (if Customer Type=Sponsor only)
Bank Account Number: The Sponsor's Bank Accout Number. (if Customer Type=Sponsor only)
EFT Effective Date: The Electric Funds Transfer effective date. (if Customer Type-Sponsor only)
EFT Type: The Electric Funds Transfer Type. Select a type from the drop down box. (ACH or Wire) (if Customer Type=Sponsor only)
Bank Account Type: Select the Bank Account Type. (if Customer Type=Sponsor only)
E-mail: Customer E-mail Address.
Phone: Customer Phone Number.
Fax: Customer Fax Number.
Level: Select Parent or Child.
Adjustment Uploaded: Select Full or Incremental. (if Customer Type=State Medicaid Program only)
845 EDI: Select to send 845 EDI. (if Customer Type=Vendor only)
849 EDI: Select to send 849 EDI. (if Customer Type-Vendor only)
After user have entered all the data for the Customer Header, press the [SAVE] button to save changes.
